In order to reduce the amount of paper work, the following certification will be <br />substituted for the actual DMR forms to cover the outfalls, which did not discharge. These outfalls include ponds T (Outfall <br />002A), B (Outfall 003A), C (Outfall 004A), D (Outfall 005A), E (Outfall 006A), F (Outfall 007A), and G (Outfall OOSA). Nate that <br />ponds A (Outfall 001 A), and H (Outfall 009A) have been deactivated following bond release, and have been removed from the <br />newly amended permit, effective January 1, 2003. Also note that Outfall 020A has not been constructed yet. <br />CERTIFICATION <br />I certify under penalty of law that l have personally examined and am /amiliar with the information submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, I believe the submitted infomtation is <br />true, accurate, and complete.
