M-97-084/Adequacy Review 10/17/97 page 3 Exhibit E -Mining and Reclamation Plan Maps The map included depicts only the pre-mining topography within the pernut boundary. Please provide additional information on the mining map as to the offsite topography to the east, plus any other pertinent details (watershed boundary, significant areas of unconsolidated material upslope, pre-existing or yet-to-be-built structures, etc.). Also please indicate the direction of mining, slopes to be maintained during mining, and locations of the various stockpiles and processing activities. Ensure that powerpoles and fences are depicted. The drainage directly to the north, whose base is near the permit area, has also deposited an alluvial fan that is partially in the permit area. It will apparently not be mined. Please indicate that material by its topography and delineate it if it is not to be disturbed. Please provide a reclamation plan map showing post-mining topography (for fully mined and partially mined scenarios), any structures to remain after reclamation (such as roads, culverted access points, fences, powerpoles, etc.). Exhibit H -Municipalities Within aTwo-mile Radius Please provide an address or other information regarding a contact for the City of Ouray. Exhibit L -Permanent Man-made Structures You have listed all the structures, but it is not clear exactly what impact mining will have upon them. This is because it is not specified in other parts of the application exactly where the various activities are to occur onsite, and because some of the structures are not depicted on the maps. If any activity related to the operation of this site is to be within 200 feet of any structure (almost unavoidable on a site this size), please provide either damage agreements or engineering evaluations, per Rule 6.3.12. Rule 6.5 - Geotechnical Stability Exhibit Review of the application materials and observations made while onsite support the Division's view that this exhibit should be submitted also. Please review Rule 6.5(1) through (3), and provide adequate analyses and evaluations as to the onsite and offsite stability as it is affected by the mining operations. Additional Items Please provide proof of individual notifications of all owners of property on the permit area or within 200 feet of the boundary (including those owners of property on the west of Highway 550). Please provide a copy of the newspaper notice that was published, and an affidavit of such publishing.
