<br /> <br />M-97-084/Adequacy Review <br />10/17/97 <br />page 3 <br />Exhibit E -Mining and Reclamation Plan Maps <br />The map included depicts only the pre-mining topography within the pernut boundary. Please <br />provide additional information on the mining map as to the offsite topography to the east, plus <br />any other pertinent details (watershed boundary, significant areas of unconsolidated material <br />upslope, pre-existing or yet-to-be-built structures, etc.). Also please indicate the direction of <br />mining, slopes to be maintained during mining, and locations of the various stockpiles and <br />processing activities. Ensure that powerpoles and fences are depicted. <br />The drainage directly to the north, whose base is near the permit area, has also deposited an <br />alluvial fan that is partially in the permit area. It will apparently not be mined. Please indicate that <br />material by its topography and delineate it if it is not to be disturbed. <br />Please provide a reclamation plan map showing post-mining topography (for fully mined and <br />partially mined scenarios), any structures to remain after reclamation (such as roads, culverted <br />access points, fences, powerpoles, etc.). <br />Exhibit H -Municipalities Within aTwo-mile Radius <br />Please provide an address or other information regarding a contact for the City of Ouray. <br />Exhibit L -Permanent Man-made Structures <br />You have listed all the structures, but it is not clear exactly what impact mining will have upon <br />them. This is because it is not specified in other parts of the application exactly where the various <br />activities are to occur onsite, and because some of the structures are not depicted on the maps. If <br />any activity related to the operation of this site is to be within 200 feet of any structure (almost <br />unavoidable on a site this size), please provide either damage agreements or engineering <br />evaluations, per Rule 6.3.12. <br />Rule 6.5 - Geotechnical Stability Exhibit <br />Review of the application materials and observations made while onsite support the Division's <br />view that this exhibit should be submitted also. Please review Rule 6.5(1) through (3), and <br />provide adequate analyses and evaluations as to the onsite and offsite stability as it is affected by <br />the mining operations. <br />Additional Items <br />Please provide proof of individual notifications of all owners of property on the permit area or <br />within 200 feet of the boundary (including those owners of property on the west of Highway <br />550). <br />Please provide a copy of the newspaper notice that was published, and an affidavit of such <br />publishing. <br />
