a) If, for any reason, the permlttee does not comply wi[1~ or will be
unable to comply with any maximum discharge limitations or
standards specified in this permit, tale permlttee shall, at a
minimum, provldo the Natcr Qunlity Control Dlvtston and E•PA w![h
the following information:
(1) A description of [he discharge and cause of noncompliance;
(li) The period of noncompliance, Including exact dates and times
and/or the anticipated time when [he discharge will return
to compliance; and
(iii) Steps being taken to reduce, eliminate, and prevent
recurrence of the noncomplying discharge.
b) The permlttee shall report the following instances
orally within twenty-four (24) hours from the time
becomes aware of [he noncompliance, and shall mall
a report within five (5) days after becoming aware

(1) Any instance of noncompliance which may end

specSFled as requiring 24 hour notification.
c) The permlttee shall report all other instances of non-compliance
not requiring 24-hour notification at the time Discharge Monitoring
Reports are submitted. The reports shall contain the information
listed in sub-paragraph (a) of this section.
