<br /> <br /> <br /> <br />1 <br /> <br /> <br />Nick Mathis <br />Site Assessment Experience <br />Mr. Mathis is an environmental geologist who has conducted numerous site assessments across <br />the country in compliance with several state agencies including the Texas Water Commission <br />(TWC), Colorado' Department of Health (CDH), New Mexico Environmental Improvement <br />District and the State of Missouri Department of Natural Resources, Division of Environmental <br />Quality. Additionally, he has conducted site assessments for the Colorado Department of Health <br />as part of their leaking underground storage tanks (LUST) Trust Fund contract. Phase I and <br />Phase II site assessments have been performed at underground storage tank (LIST) facilities, <br />industrial facilities, retail facilities, vacant land, residential azeas and abandoned landfills. The <br />assessments have included defining and delineating petroleum hydrocarbon contamination a~ <br />hazardous waste contamination. <br />Mr. Mathis has managed and performed over 200 Phase I and Phase II site assessments in 25 <br />' states. Each assessment requires knowledge of state and federal hazardous waste regulations and <br />databases along with site reconnaissance, interviews, regulatory interface and records review. <br />' A sample of the types of Phase II assessments that Mr. Mathis has performed includes retail <br />' UST facilities for Exxon, Phillips Petroleum, Total Petroleum and Diamond Shamrock. <br />Additionally, Phase II assessments have been conducted of lazge industrial facilities such as the <br />General Motors facility in Dayton, Ohio and Ball Industries in El Segundo, California. These <br />' assessments were subsurface investigations which consisted of investigating and identifying <br />potentially hazardous materials. <br />' Mr. Mathis has conducted soil, asbestos, drinking water, surface water, radon, lead in paint and <br />groundwater sampling. In addition, he has installed groundwater monitoring wells, conducted <br />aquifer pump and slug tests, analyzed and evaluated field and analytical data and prepared the <br />site assessment reports. <br />Mr. Mathis has performed a wide variety of other environmental projects involving UST remov- <br />als and closures, remedial activities at UST and hazazdous waste sites, hazardous materials <br />inventories and disposal and preparation of Spill Prevention Control and Countermeasure Plans <br />' (SPCC). <br /> <br /> <br /> <br />' !1/95.~SA <br />
