Request for laboratory QC documentation. <br />' 8. Types of containers and numbers. <br />' 9. Sample identification and matrix <br /> 10. Preservative method <br />' <br /> 11. Date results are required <br />' 12. Signatures, dates, and times for aII persons involved in possession of samples. <br />' 13. Method of shipment, name of carrier, and carrier ttacl®g number. <br />14. Comments <br />The original chain of custody v+ID accompany the samples thronghoat shipment.
