Representative Sampling <br />!~ <br />PART I <br />Page 10 <br />Permit No. COG-850000 <br />Samples and measurements taken for the purpose of monitoring shall be <br />representative of the monitored discharge. <br />2. Reporting <br />Monitoring results for each calendar quarter are due by the 28th day of <br />the following month. The results shall be summarized and reported on <br />applicable discharge monitoring report forms CEPA Form 3320-1), <br />postmarked no later than the due date. If no discharge occurs during <br />the reporting period "No Discharge' shall be reported. Duplicate <br />signed copies of these and all other reports required herein, shall be <br />submitted to the Regional Administrator and the State at the following <br />addresses: <br />Colorado Department of Health <br />Water Quality Control Division <br />4210 East 11th Avenue <br />Denver, Colorado 80220 <br />Attention: Permits and Enforcement <br />D. Environmental Protection Agency <br />One Denver Place <br />Suite 500 <br />999 18th Street <br />Denver, Colorado 80202-2405 <br />Attention: Water Management Division-Compliance Branch <br />SWM-C <br />Failure to submit the reports shall constitute a violation of the <br />permit and may constitute grounds for revocation of the permittee's <br />authority to discharge under the permit. <br />3. Test Procedures <br />Test procedures for the analysis of pollutants shall conform to <br />regulations published pursuant to Section 304 (b) of the Clean Water <br />Act, and Colorado State Effluent Limitations (10.1.5), under which such <br />procedures may be required. <br />
