During this period, ponds A (Outfall 001), B (Outfall 003), E (Oudall 006), and H (Oudall 009) discharged to Foidel <br />Creek. Outfalls 001, 003, and 009 discharged each week of the period. Outfall 006 discharged during the first 4 weeks of the <br />period. No excursions occurred during the period. <br />Outfalls 002, 004, 005, 007, and 008, did not discharge; and therefore had no chemical tests during this period. Outfall 020 <br />has not been constructed yet; therefore there is no discharge to enable chemical testing to date. Points 010, 016 and 017 <br />have been eliminated from this permit, In order to reduce the amount of paper work, the following certification will be <br />substituted for the actual DMR forms to cover the oudalls, which did not discharge. <br />CERTIFICATION <br />1 certify under penalty of law that 1 have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry o(those individuals responsible for obtaining the information, !believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are significant penalties for submitting false information, including the <br />possibility of fine and imprisonment. See 18 U.S.C. § 1001 and 33 U.S.C. § 1319.
