(MonitornQ and Reporting) of the discharge permit for your facility requires that the discharge be <br />monitored at specified frequencies ar:d that the analytical results be reported to this Division on Discharge <br />Monitoring Report (DMR) Form 3320-I. Failure to submit DMRs within the required timeframe constitutes a <br />violation of your permit. <br />A review of our records indicates that we have not received the DMRs for outfall(s) 001 A, 002A for the <br />period(s) of 07/01/2003 - 09/30/2003. If your records indicate that the DMRs for these outfalls were submitted <br />then please check with me at (303) 692-3595 to make sure that they have been received by this office. If we <br />have not received them then we ask that you send a copy as well as a copy of the return receipt if they were sent <br />by certified mail.
