PART 1 <br />Page xvii <br />Permit No. CO-0043648 <br />I5. 'Thirty (30) day average" means, except for fecal coliform bacteria, the arithmetic mean of all samples collected <br />during a thirty (30) consecutive-day period. For fecal coliform bacteria, it is the geometric mean of all samples <br />collected in a thirty (30) day period. The permittee shall report the appropriate mean of all self-monitoring sample <br />data collected during [he calendar month on the Discharge Monitoring Reports. Samples shall not be used for more <br />than one (1) reporting period. <br />16. "Twenty four (24) hour composite" sample is a combination of a[ least eight (8) sample aliquots of at least 100 <br />milliliters, collected at equally spaced intervals during the operating hours of a facility over atwenty-four (?A) hour <br />period. For volatile pollutants, aliquots must be combined in the laboratory immediately before analysis. The <br />composite must be flow proportional; either the time interval between each aliquot or the volume of each aliquot must <br />be proportional to either the wastewater or effluent flow at the time of sampling or the total wastewater or effluent <br />Flow since the collection of the previous aliquot. Aliquots may be collected manually or automatically. <br />17. 'Visual" observation is observing the discharge to check for the presence of a visible sheen or floating oil. <br />18. "Water Quality Control Division" or 'Division' means the state Water Quality Control Division as established in 25- <br />8-101 et al. <br />D. REPORTING <br />1. Sienatorv Requirements <br />All reports required for submittal shall be signed and certified for accuracy by the permi[[ee in accord with the <br />following criteria: <br />a) In the case of corporations, by a principal executive officer of at least the level of vice-president or his or her <br />duly authorzed representative, if such representative is responsible for the overall operation of [he facility from <br />which the discharge described in the form originates; <br />b) In the case of a partnership, by a general partner: <br />c) In the case of a sole proprietorship, by the proprietor; <br />d) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking elected <br />official, or other duly authorized employee. <br />2. Certification <br />Any person signing a document for submittal shall make the following certification: "I certify under penalty of law <br />that this document and all attachments were prepared under my direction or supervision in accordance with a system <br />designed to assure that qualified personnel properly gather and evaluate the information submitted. Based on my <br />inquiry of the person or persons who manage the system, or those persons directly responsible for gathering, the <br />information submitted is, to the best of my knowledge and belief, true, accurate, and complete. [ am aware [hat there <br />are significant penalties for submitting false information, including the possibility of fine and imprisonment for <br />knowing violations." <br />INDaP aerv~ <br />
