This permit Is Issued in reliance upon the accuracy and completeness of information supplied by the applicant and <br />is cdnditloned upon conduct of the activity, a construction, intallatlon and oparetlon d the source, in accordance <br />with this informatOn and with representation made by the applicant ar applinnhs agents. It is valid only for the <br />equipment and aperatiorts or actvrty specifically identified on the permit. <br />2. Unless spec(fically stated otherwise, the general and specific condition contained In this permit have been <br />determined Dy the APCD to [1e necessary to assure compliance with the provisions of Sectlan 257.7fa.5(7)(a), <br />C.R.S. <br />3. With raspeet to a portable source which K moved to a new location, a copy of the <br />Relouatlan Notice (required by law m ba submllted to the APCD whenever a portable source is relocated) should <br />tw attachetl to this permit. The permit may ba rr~ssued to a new owner by the APCD as provided in AQCC <br />Regulation No. 3, Part B, Section ttl.9, upon a request for transfer of ownership acrd the submittal of a revised APEN • <br />and the required tea. <br />5. Issuance (initial approval) of an amissien permit does not provide "final atrthority for this activity ar o0sretlon of this <br />seurre. Final approval of the penult must be seared from the APCD In writing in accordance with the provisions <br />of 2117-1f4.5(12)(e) C.RS. and AOCC Regulation No. 3, Part B, Section Iv.H. Final approval cannot be gramed <br />until the operation or acYivlty commences and has been vartfied by the APCD as conforming in all respects with the <br />conditions of the permit. If a source or activity Is to ha <br />discominued, the owner must notify the DIVIsIOn in writing requesting a rancellatlon of the permR.
