Please have <br />the Assignment form signed by Mr. Peterson, have his signature notarized and <br />then have the form acknowledged by an authorized individual from your bank. <br />Please return this original document along with the original Certificate of <br />Deposit and signature card. Upon receipt of these documents, I will have our <br />Division Director, Michael B. Long, sign the signature card on June 12, 1995 <br />and return to your bank immediately. (He is out of town until that date.) <br />It is required by Colorado State statutes governing mined land reclamation <br />that our Division shall have the original, properly executed bonding documents <br />before a mining permit can be issued to the applicant/permittee; which in this <br />case is Merlin J.
