(of your choice) <br />- Analysis type (see below) <br />April and October Sample - Tvoe G-SAR <br />July Samples - Tvoe F-SAR <br />• If well is dry, has too little water to sample, or is frozen <br />- Well no. <br />- Date <br />- Time <br />- Water level or level to bottom of well <br />- Note any comments; well dry, too little water to sample, <br />well frozen, unusual circumstances, damage to well, or <br />maintenance required <br />23-188 <br />
