<br />-5- <br />EXHIBITS A-J <br /> <br />Two (2) complete copies of the application mJSt be submitted. One complete copy of an <br />application consists of the application form and the set of maps and exhibits referenced <br />below as Exhbits Awl. Each exhibit must be presented as a separate section. Begin each <br />exhibit on a new page. Pages should be numbered consecutively for ease of reference. <br />If separate bound doaments are used as appendices, please reference these by name in <br />the exhibit. <br />EXHIBIT A - Legal Description <br />The legal description must identify the affected land, specify the acreage of affected <br />land and be adequate for field location of the property. It should be a description by <br />reference to township, range, and section, to at least the nearest quarter-quarter <br />section. A metes and bounds survey description is also acceptable. Where applicable, <br />the street address or lot number(s) may be used. This information is available from the <br />County Assessor's office or maps from the U.S. Geol~~gical Survey (USGS). NOTE: If you <br />improperly locate your operation in the field, and •is a result mine outside the area <br />delineated by this permit application, it could be :onsidered to be a violation of <br />34-32-123 or 34-32-124, C.R.S. of the Mined Land Re~:lamation Act. <br />EXHIBIT B -Site Description <br />Items 1-3 below must be addressed to the extent necessary to demonstrate compliance with <br />the applicable performance standard requirements of Rule 6. At a minimum, you should <br />include the following information: <br />1. A description of the vegetation and sail characteristics in the area of the <br />proposed operation. The local office of the Soil Conservation Service (SCS) may <br />provide you with this information as well as recommendations far Exhibit D - <br />Reclamation Plan. Contact the Regional SCS office in Denver at (303) 964-0295 or <br />the Mined Land Reclamation Division for assistance in locating the office nearest <br />Yom: <br />2. Identify any permanent man-made structures within 200 feet of the affected <br />area and the owner of each structure. Permanent man-made structures include <br />houses, barns, fences, above or below ground utilities, irrigation ditches, <br />maintained or public roads, bridges, wells, pipelines, water storage impoundments, <br />railroad tracks, cemeteries, and communication antennas, etc. Each structure <br />should be located on Exhibit E - Map. <br />3. A description of the water resources in the area of the proposed operation. <br />Identify any streams, springs, lakes, stockwater ponds, ditches, reservoirs, and <br />aquifers which would receive drainage directly from the affected area. Provide any <br />information available from publications or monitoring data on flow rates, water <br />table elevations and water quality conditions. <br />
