Laboratories, Inc.
Steamboat Springs, CO LJ4Bi (800) 334-5493
ACZ Laboratories, Inc. (ACZ) received 1 surface water sample trom Midas Mining Co. on October 11, 2001. The sample
was received in good condition. Upon receipt, the sample custodian removed the sample from the cooler, inspected the
contents, and logged the sample into ACZ's computerized Laboratory Information Management System (LIMB). The sample
was assigned ACZ LIMB project number L34333. The custodian verified the sample information entered into the computer
against the chain of custody (COC) forms and sample bottle labels.
All analyses were performed within EPA recommended holding times.
This sample was analyzed for inorganic parameters. The individual methods are referenced on both, the ACZ invoice and
the analytical reports. The following anomaly was identified during the analysis of this sample:
1. The Dissolved Silver is estimated due to the precipitation of Silver in the stock standard (PCN 14833) for the
LCS/ICV/CCV. The recoveries are low for these OC samples. The data was accepted since the problem with the standard
was verified by ICP analysis. Anew stock solution has been ordered.
REPAD.02.11.00.01
L34333: Page 2 of 7
Midas Mining Co. November 07, 2001
Project: L34333
