This report is filed in accordance with Rule 2.02.6(3)(b) of the Regulations of the
Colorado Mined Land Reclamation Board for Coal Mining adopted in accordance with
the Colorado Surface Coal Mining Act, C.R.S. 34-33-101 et sea. The report is to be
submitted to the Division within 30 days after the end of die period of exploration
specified in the approved Notice of Intent, as amended.

a,) The final locations of all holes drilled during this project;
b, The locations of all roads modified or constructed for the
project; and
c.) Ali areas siDnificantly disturbed by the operator,
2. Have all disturbed areas, including drill holes, been reclaimed in
accordance with the approved plan and the above-referenced regulations? YES
3. Were any newly constructed project roads retained for the landowner? NO
If yes, a) attach a letter from the landowner that grants consent, b) indicate
the road location on a map, and c) describe the approximate specifications of
the road (width, length, surface material used).

procedures that were used.
