It was mentioned at the informal conference that a weigh station (scale) will be part of the <br />equipment within the permit area. This was not included in the application. If you intend to <br />have a weigh station in the permit area it must be shown on all the maps and you must <br />provide enough information (structure dimensions, if concrete will be used as a foundation <br />/ for the building, etc.) to determine reclamation costs. <br />/ 2. There was mention of paving the access road. This is not included in the application. Please <br />provide a narrative explaining exactly what you plan to do. <br />~~ 3. Pursuant to my pre-operation inspection report, dated February 17, 1994, you must mitigate <br />the structures that are located within 200' of the proposed affected area. <br />~4. The State Historical Society must provide the operator with an exemption letter if an <br />archeological survey is not required. Please contact Jim Green for a determination of their <br />requirements. Send a copy of all correspondence to the DMG. <br />I am enclosing copies of the objection letters I received at the informal conference. Of note, <br />are the concerns about radon gas emissions (Todd Richardson), noise abatement under state statute <br />25-12-101 (Cindy Bowen, County Commissioner) and Stuart Chappell's concern about a culvert on <br />the irrigation ditch. Please address these concerns and copy the DMG with all correspondence. <br />Pursuant to Rule I.5 your response to the above noted adequacy requirements will constitute <br />changes to your application. A copy of the changes must be given to the County Clerk and <br />Recorder and proof that the Clerk and Recorder received the changes must be sent to the Division. <br />Within five (5) working days after receiving the amended f'ding, the Division shall set a new date <br />for the consideration of the application. <br />
