If as authorization under paragraph a) of <br />this section is no longer accurate becauea a different individual or <br />position has responsibility for the overall operation of the <br />facility, a new authorization aatisfyinq the requirsmeats of <br />paragraph a. of this section must be submitted to the Division, prior <br />to or together with any reports, information, or applications to be <br />signed by an authorized represeatatiw. <br />c) Grtilicatioa. Any person signing a document under paragraph a) of <br />thin section shall make the following certification: <br />'I certify under penalty of law that this document end all <br />attachments ware prepared under my direction or supervision in <br />accordance with a system designed to assure that qualified personnel <br />properly gather and evaluate the information submitted. Based on my <br />inquiry of the person or persona who manage the system, or those <br />persona directly responsible for gathering, the information submitted <br />ie, to the best of my knowledge and belief, true, accurate, and <br />complete. i am aware that there are significant penalties for <br />submitting false information, including the possibility of fine and <br />imprisonment for knowing violations." <br />• PART II <br />A. MANAGEMENT REQUIREMENTS <br />1. Chance in Diacharce <br />The permittee shall inform the Division (Permits and 8nforcement <br />Section) in writing of any intent to construct, install, or alter any <br />proceae, facility, or activity that ie likely to result in a new or <br />altered discharge, and shall furnish the Division each plane and <br />specifieationa which the Division deems reasonably neceaaary to evaluate <br />the effect on the discharge and receiving stream. <br />The permittee shall submit this notice within two (2) weeks after making <br />a determination to perform the type of activity referred to in the <br />preceding paragraph. Process modifications include, but are not limited <br />to, the introduction of any new pollutant not previously identified in <br />the permit, or any other modifications which may result in a discharge <br />of a quantity or quality different from that which was evaluated in the <br />drafting of the permit including subsequent amendments. Following such <br />notice, the permittee shall be required to submit a new CDPS application <br />and thin application may be proceaeed ae an application for an <br />individual permit if the application chews that the new discharge ie <br />ineligible for coverage under the general permit.
