R~ortinc - Process Water Monitoring results obtained during the previous 3 months ^hall be summarized and reported on Division approved discharge monitoring report forma, postmarked no later than the 28th day of the month following the completed calendar quarter. For example, for the let quarter (January, February, and March) the quarterly report must be received at this office postmarked no later than April 28. Reoort ins - Stormwat er Annual Reoort The permittee will ba required to submit an annual report, covering January 1 through December 31 of each year, on the overall compliance with the SWMP. The annual report will contain, at a minimum: a) Name of permittee, address, phone number, and permit certification number. b) A report on the facility's overall compliance with the SWMP.
