Preliminary review of the information received <br />determined that the following items must be received before the Division can <br />consider your application as being submitted and technical review can begin. <br />I) The application fee for your 110 permit has been iacreased effective July <br />1s°,2007 to $1258.00. Please submit an additional fee in the amount of <br />$164.00. <br />You have (60) days from the above date to submit all necessary <br />documents that the Office needs for an application to be considered <br />filed. If, at the end of the (60) days from the above date to submit <br />all necessary documents that the Office needs for an application to be <br />considered filed.
