Preliminary <br />review of the information received detertined that the following items must be received before the Division can consider your <br />application as being submitted and technical review can begin: <br />APPf .iC ATTON FORMS page 7: The Division requves a corporate seal be affixed to the signature page. <br />F.X1fIRJT f -Prnnf of Filipg with Cnunly Clerk nle 6 7.91: provide an affidavit or receipt indicating the date on which <br />the application was placed with the local County Clerk and Recorder Also, refer to Rule 1.6.2(1)(c). Need proojof receipt <br />from County Clerk, not from the County Planning Department. <br />F.XHfIt1T .f -Prnnf of Mailing NnNces of Permit A~nlieaNnn /Role 67.101: Include proof of mailings of the "Notice of <br />Permit Application" sent to the County Boazd of Commissioners, in the application, by submitting the date-stamped receipt <br />for Certified Mailing or adate-stamped copy of the notice form with signature acknowledging personal service and receipt by <br />the appropriate boazd. Also, refer to Rule 1.6.2(lxa). <br />Your application will not be considered filed until the information listed above is received and found sufficient to begin our review. A <br />decision date will be established 30 days from the date of receipt of all of the requested information. Addifionally, if you have already <br />published notice you will need to republish notice, after the Division considers the application submitted. Republication must occur <br />once within ten (10) days of the submittal date. We will notify you when you should initiate republication of your notice. The final <br />date for receiving comments is ten (10) days after the date of publication or the next regulaz business day. <br />You have sixty (60) days from April 24, 2006 to submit all necessary documents that the Office needs for an application to be <br />considered filed. If, at the end of the sixty day period, the application has not been determined to be filed with the Otice, the Office <br />shall deny the application and terminate the application file. <br />This letter shall not be consWed to mean that there are no other technical deficiencies in your application.
