GENERAL MONITORING, SAMPLING, AND REPORTING REQUIREMENTS <br />1. RrnrtineR@portina_~~fData <br />Reporting of the data gathered in compliance with Part I.B.1 shall be on a monthly basis. Reporting of all data gathered shall <br />comply with the requirements of Part I.F. (General Requirements). Monitoring results shall be summarized for each month and <br />reported on Division approved dischazge monitoring report (DMR) forms (EPA form 3320-1). The forms shall be mailed to the <br />agencies listed below so they are postmazked no later than the 28th day of the month following the end of each calendar month. <br />If no discharge occurs during the reporting period, "No Discharge" shall be reported. <br />The DMR forms consist of four pages -the top "original" copy, and three attached no-carbon-required copies. After the DMR <br />form has been filled out and signed, the four copies must be sepazated and distributed as follows: <br />The first original signed copy of each dischazge monitoring report (DMR) shall be submitted to the Division at the following <br />address: <br />Colorado Deparhnent of Public Health and Environment <br />Water Quality Control Division <br />WQCD-P-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80246-1530 <br />The first duplicate signed copy of each discharge monitoring report (DMR) shall be submitted to the following agency: <br />U.S. Environmental Protection Agency Region VIII <br />Technical Enforcement Program (8ENF-T) <br />Office of Enforcement, Compliance Assistance, and Environmental Justice <br />999 18th Street, Suite 300 <br />Denver. CO 80202-2466 <br />The third and fourth copies are for the pernrittee records. The Dischazge Monitoring Repor[ forms shall be filled ou[ accurately <br />and completely in accordance with requirements of this pemmit and the instructions on the fom~s. An authorized person as <br />identified in Patt I.D.6 shall sign them. <br />Calculations for all limitations, which require the averaging of meastuements, shall utilize an arithmetic mean unless otherwise <br />specified by the Division in the pemut. <br />2. Renrecentative Rami h-ng <br />Samples and measurements taken as required herein shall be representative of the volume and nature of the monitored <br />discharge. All samples shall be taken at the monitoring points specified in this pemvt and, unless otherwise specified, before <br />the effluent joins or is diluted by any other wastestream, body of water, or substance. As indicated in Section B.1 of this pemmit, <br />no special sampling restrictions related to stormwater apply to Outfa11001A. However, the data that are collected must provide <br />an accurate representation of the quantity and quality of the water flowing past the point. If saznples aze collected when duect <br />surface nmoff resulting from rainfall is observed in the flow being monitored, this must be noted and reported on the monthly <br />DMR. Monitoring points shall not be changed without notification to and approval by the Division. <br />If the pemrittee monitors at the point of dischazge any pollutant limited by the permit more frequently than required by the <br />permit, using approved test procedures or as specified in the permit, the result of this monitoring shall be included in the <br />calculation and reporting of data to the Division. <br />
