GENERAL MONITORING, SAMPLING, AND REPORTING REQUIREMENTS 1. RrnrtineR@portina_~~fData Reporting of the data gathered in compliance with Part I.B.1 shall be on a monthly basis. Reporting of all data gathered shall comply with the requirements of Part I.F. (General Requirements). Monitoring results shall be summarized for each month and reported on Division approved dischazge monitoring report (DMR) forms (EPA form 3320-1). The forms shall be mailed to the agencies listed below so they are postmazked no later than the 28th day of the month following the end of each calendar month. If no discharge occurs during the reporting period, "No Discharge" shall be reported. The DMR forms consist of four pages -the top "original" copy, and three attached no-carbon-required copies. After the DMR form has been filled out and signed, the four copies must be sepazated and distributed as follows: The first original signed copy of each dischazge monitoring report (DMR) shall be submitted to the Division at the following address: Colorado Deparhnent of Public Health and Environment Water Quality Control Division WQCD-P-B2 4300 Cherry Creek Drive South Denver, Colorado 80246-1530 The first duplicate signed copy of each discharge monitoring report (DMR) shall be submitted to the following agency: U.S. Environmental Protection Agency Region VIII Technical Enforcement Program (8ENF-T) Office of Enforcement, Compliance Assistance, and Environmental Justice 999 18th Street, Suite 300 Denver. CO 80202-2466 The third and fourth copies are for the pernrittee records. The Dischazge Monitoring Repor[ forms shall be filled ou[ accurately and completely in accordance with requirements of this pemmit and the instructions on the fom~s. An authorized person as identified in Patt I.D.6 shall sign them. Calculations for all limitations, which require the averaging of meastuements, shall utilize an arithmetic mean unless otherwise specified by the Division in the pemut. 2. Renrecentative Rami h-ng Samples and measurements taken as required herein shall be representative of the volume and nature of the monitored discharge. All samples shall be taken at the monitoring points specified in this pemvt and, unless otherwise specified, before the effluent joins or is diluted by any other wastestream, body of water, or substance. As indicated in Section B.1 of this pemmit, no special sampling restrictions related to stormwater apply to Outfa11001A. However, the data that are collected must provide an accurate representation of the quantity and quality of the water flowing past the point. If saznples aze collected when duect surface nmoff resulting from rainfall is observed in the flow being monitored, this must be noted and reported on the monthly DMR. Monitoring points shall not be changed without notification to and approval by the Division. If the pemrittee monitors at the point of dischazge any pollutant limited by the permit more frequently than required by the permit, using approved test procedures or as specified in the permit, the result of this monitoring shall be included in the calculation and reporting of data to the Division.
