Foothills H Michael B. long <br />~' Division Director <br />Longmont, CO 80503 <br />Re: Starks Quarry, File No. M-2001-114, Receipt of 110 Construction Materials Reclamation Permit <br />Application Package <br />Dear Mr. Loukonen: <br />On November 29, 2001, the Division of Minerals and Geology received your the 110 Construction Materials <br />Reclamation Permit Application package for the Starks Quarry, File No. M-2001-114. Preliminary review of the <br />information received determined that the following items must be received before the Division can consider your <br />application as being submitted and technical review can begin: <br />APPLICATION FORM: Item 1, Page 1 on the Application Form lists the Applicant/Operator Name as <br />Loukonen Bros. Stone. Item 14, Page 3 on the Application Form lists the Applicant/Operator Company <br />Name as Loukonen Bros. Stone. Page 6 shows the Applicam/Operator name listed as Loukonen Bros. <br />Stone. The Corporate Seal on Page 6 shows the Corporation Name as Loukonen Bros. Stone Company. <br />The notary notarized Michael Loukonen's signature as General Manager of Loukonen Stone Co. Exhibit <br />G -Legal Right to-Enter shows that the lease is between Douglas E. Starks and Loukonen Bros. Stone <br />Company. The ApplicantJOperator name must be consistent throughout the entire applicafion <br />documentation. Please make all of the necessary changes to the ApplicanUOperator name (your original <br />application form is enclosed so that you can make the changes, you will need to send replacemem pages <br />for any exhibits that need to be corrected). <br />APPLICATION FORM -Item 9 -General Location Information: The Division request a complete <br />location description be given for Item 9 of the Application form. The information that was enclosed in the <br />form submitted only states that the site is 1 mile east. It doesn't state the closest Town or Municipality. <br />EXHIBIT E -Mao (Rule 6.3.5): The only map that was enclosed for this exhibit was a location map. <br />(1) In addition to the requirements of 6.2.1(2), you must provide a map that clearly describes the features <br />associated with the mining plan and the components of the Reclamation Plan. Include one map for the <br />mine plan and one map for the Reclamation Plan. The map(s) must be drawn to a scale no smaller than <br />appropriate to clearly show all elemeals that are required to be delineated by the Act and these Rules; <br />show a north arrow, note any section comers adjacent to the proposed operation, and indicate the date <br />illustrated.
