A total of fourteen (19) alluvial groundwater monitor wells will be
installed on a total of thirteen (13) individual sites, as indicated
on the attached Map 1. Surface land ownership of the well sites and
access to the sites is as indicated below:

Land Co., and the various land owners are attach ed.
4. Attach a description of the land to be affected (Rule 2.02.2(2)(c)). For example: The
condition of the topsoil, the general hydrology, type of vegetation and topography.
The general area included within the NOI boundary is characterized
by topography of high and low relief. Seven drainages are found
within the NOI area. Five 1~` and 2ntl order tributaries to the Yampa
River drain northward from the Williams Fork Mountains toward the
Yampa River. The streams have perennial and intermittent sections.
Individual well sites are all flat-lying, located near well-traveled
county and private roads, are overlain by silty topsoil, and
vegetated with low grasses. Photographs of each of the well sites
are attached (photos are recent, taken May 7`", 1998).
