Box 1720 Canon City, Co 81215-1720 Re: Permit # M-2000-151, Adequacy concems Dear Miss Bellantoni, The Division has reviewed your 112 construction permit application # M-2000- l51 and has the following concerns. Please be advised that your application may be denied on 3-21-01, unless the following issues are addressed to the satisfaction of the Division. I) EXHIBIT "A" Legal Description, Adequate )n EXHIBIT "B" Index Map, Adequate III) EXHIBIT "C" Pre- mining and Mining plan maps of affected land. A) Rule 6.4.3 (b) requires the operator to provide on a map the name and location of all creeks, roads, buildings, oil and gas wells and lines, power and communication lines on the area of affected land and within two hundred (200) feet of al] boundaries of such area. Is the operator in compliance with the above rule ? P/) EXHIBIT "C" Mining Plan A) The purpose of the mining plan is to describe how mining will affect the permit area for the duration of the operation. The description of the mining plan must be adequate to satisfy the requirements of 3.1 and demonstrate compliance with Rue 3. Please provide the required in accordance with the above rule. V) EXHIBIT "G" Water information A) The operator states "Five wells exist with in 600 feet of the permit area. The static water levels range from 10 feet to 45 feet. The wells lie with in the permit area and belong to the property owner and mining will not impact these wells". 1) How close is the mining operation going to be to these wells ?The Division requires a
