Please be advised that your application may be denied on 3-21-01, unless the <br />following issues are addressed to the satisfaction of the Division. <br />I) EXHIBIT "A" Legal Description, Adequate <br />)n EXHIBIT "B" Index Map, Adequate <br />III) EXHIBIT "C" Pre- mining and Mining plan maps of affected land. <br />A) Rule 6.4.3 (b) requires the operator to provide on a map the name and location of all creeks, <br />roads, buildings, oil and gas wells and lines, power and communication lines on the area of <br />affected land and within two hundred (200) feet of al] boundaries of such area. Is the operator in <br />compliance with the above rule ? <br />P/) EXHIBIT "C" Mining Plan <br />A) The purpose of the mining plan is to describe how mining will affect the permit area for the <br />duration of the operation. The description of the mining plan must be adequate to satisfy the <br />requirements of 3.1 and demonstrate compliance with Rue 3. Please provide the required in <br />accordance with the above rule. <br />V) EXHIBIT "G" Water information <br />A) The operator states "Five wells exist with in 600 feet of the permit area. The static water <br />levels range from 10 feet to 45 feet. The wells lie with in the permit area and belong to the <br />property owner and mining will not impact these wells". <br />1) How close is the mining operation going to be to these wells ?The Division requires a <br />
