Reoortine to Municipality Any permitted Facility discharging to a municipal storm sewershall provide the municipality with a copy ofthe permit application, upon request. A copy of the S WMP shall also be provided to the municipality upon request. 8. Special Monitorine TDS monitoring is only required for facilities discharging in the Colorado River Basin. TDS may be measured as electrical conductivitywhere asatisfactory conelationwith TDS has been established based upon a minimum offive (5) samples. See the individual Cerification Rationale for a determination as to whether monitoring is required. b. Phosphorus monitoring is required only for facilities which discharge into the following drainage basins: Cherry Creek basin, Chatfield Reservoir downstream ofthe USGS gage at Waterton and on Plum Creek, Dillon Reservoir basin (i.e., Ten Mile Creek, Snake River, Blue River, all tributaries to the Dillon Reservoir), and Bear Creek basin. See the individual Cerification Rationale for a determination as to whether monitoring is required. 9. Sienatorv Requirements a. All reports required for submittal shall be signed and certified for accuracy by the permittee in accordance with the following criteria: 1) In the case of corporations, by a principal executive officer of at least the level ofvice-president or his or her duly authorized representative, ifsuchrepresentative isresponsible forthe overnlloperation ofthe facility from which the discharge described in the form originates; 2) In the case of a partnership, by a general partner; 3) In the case of a sole proprietorship, by the proprietor; 4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking elected official, or other duly authorized employee. b. Changes to authorization. If an authorization under paragraph a) of this section is no longer accurate because a different individual or position has responsibility for the overall opern[ion of the facility, a new authorization satisfying the requirements of parngraph a} of this section must be submitted to the Division, prior [o or together with any reporu, information, or applications to be signed by an authorized representative. c. Certification. Any person signing a document under paragraph a) ofthis section (except for the SWMP certification) shall make the following certification: "I certify under penalty of law that this document and all attachments were prepared under my direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best ofmy knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting fatse information, including the possibility of fine and imprisonment for knowing violations." E. DEFINITIONS I. A "continuous" measurement, for flow monitoring requirements, is a measurement obtained from an automatic recording device, which continually measures flow. 2. A "grab" sample, for monitoring requirements, is a single "dip and take" sample. 3. An "in-situ" measurement, for monitoring requirements, is defined as a single reading, observation, or measuremem taken in the field of the point of discharge. 4. An "instantaneous" measurement, for monitoring requirements, is a single reading, observation, or measurement performed on site.
