Reoortine to Municipality <br />Any permitted Facility discharging to a municipal storm sewershall provide the municipality with a copy ofthe permit application, <br />upon request. A copy of the S WMP shall also be provided to the municipality upon request. <br />8. Special Monitorine <br />TDS monitoring is only required for facilities discharging in the Colorado River Basin. TDS may be measured as electrical <br />conductivitywhere asatisfactory conelationwith TDS has been established based upon a minimum offive (5) samples. See <br />the individual Cerification Rationale for a determination as to whether monitoring is required. <br />b. Phosphorus monitoring is required only for facilities which discharge into the following drainage basins: Cherry Creek <br />basin, Chatfield Reservoir downstream ofthe USGS gage at Waterton and on Plum Creek, Dillon Reservoir basin (i.e., Ten <br />Mile Creek, Snake River, Blue River, all tributaries to the Dillon Reservoir), and Bear Creek basin. See the individual <br />Cerification Rationale for a determination as to whether monitoring is required. <br />9. Sienatorv Requirements <br />a. All reports required for submittal shall be signed and certified for accuracy by the permittee in accordance with the following <br />criteria: <br />1) In the case of corporations, by a principal executive officer of at least the level ofvice-president or his or her duly <br />authorized representative, ifsuchrepresentative isresponsible forthe overnlloperation ofthe facility from which the <br />discharge described in the form originates; <br />2) In the case of a partnership, by a general partner; <br />3) In the case of a sole proprietorship, by the proprietor; <br />4) In the case of a municipal, state, or other public facility, by either a principal executive officer, ranking elected <br />official, or other duly authorized employee. <br />b. Changes to authorization. If an authorization under paragraph a) of this section is no longer accurate because a different <br />individual or position has responsibility for the overall opern[ion of the facility, a new authorization satisfying the <br />requirements of parngraph a} of this section must be submitted to the Division, prior [o or together with any reporu, <br />information, or applications to be signed by an authorized representative. <br />c. Certification. Any person signing a document under paragraph a) ofthis section (except for the SWMP certification) shall <br />make the following certification: <br />"I certify under penalty of law that this document and all attachments were prepared under my direction or supervision in <br />accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted. <br />Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the <br />information, the information submitted is, to the best ofmy knowledge and belief, true, accurate, and complete. I am aware <br />that there are significant penalties for submitting fatse information, including the possibility of fine and imprisonment for <br />knowing violations." <br />E. DEFINITIONS <br />I. A "continuous" measurement, for flow monitoring requirements, is a measurement obtained from an automatic recording device, <br />which continually measures flow. <br />2. A "grab" sample, for monitoring requirements, is a single "dip and take" sample. <br />3. An "in-situ" measurement, for monitoring requirements, is defined as a single reading, observation, or measuremem taken in the field <br />of the point of discharge. <br />4. An "instantaneous" measurement, for monitoring requirements, is a single reading, observation, or measurement performed on site. <br />
