The Division of Minerals and Geology (Division) is required <br />to make an approval or denial decision no later than December 29, 2000, therefore, your <br />response to the following adequacy review concerns should be submitted to the Division as soon <br />as possible. If you feel more time is needed to complete your reply, the Division can grant an <br />extension to the decision date. This will be done upon receipt of a written waiver of your right to <br />a decision by December 29, 2000 and request for additional time. This must be received no later <br />than the deadline date. <br />p~ ~ ~.,e ~o IZ -oa <br />1. As required by Rule 1.6.2 (1)(d), please submit the proof of publication of the public notice <br />circulated in the local newspaper. Proof of notice may be return receipts of a Certified <br />Mailing or by proof of personal service. <br />2. Rule 6.3.3(b) requires the applicant to specify how topsoil will be stabilized with a vegetative <br />cover if the pile is in place for over one yeaz. Please specify the topsoil pile seed mixture that <br />will be used to stabilize the pile from wind and water erosion. <br />3. Please provide the Division a general location map of the proposed operation. The vicinity <br />(/'1 ~ ~~ map on Exhibit C & Exhibit E does not adequately show how to access the site from any <br />ae'r' major roads or towns in Logan County. <br />4. Rule 6.2.1 specifies the general requirements of Exhibits and Maps to contain the following; <br />V - ` Il show the name of the applicant, <br />,~' • must be signed by a registered land surveyor, engineer or other qualified person, <br />~ give the date prepazed, and <br />• include a scale, and <br />• include a field locatable point. <br />Please revise the Mine Plan and Reclamation Plan Maps to include this information. <br />
