7) Per Rule 6.2.1(2)(b), all maps must be signed by the qualified person preparing them. Staff notes <br />no signatures on the submitted maps. Please provide maps with required signatures and any <br />corrections ifnoted. <br />8) A city water line is noted within text near the edge of the proposed permit area. Staff however <br />can not find the line on the Mining Map or noted as a man made feature within 200 feet. Please <br />clarify that all utilities which by rule are considered man made structures are accounted for <br />within the 200 foot area and that proper notice as required will be forth coming. <br />9) Exhibit G, Right of Entry, is insufficient. Please provide a deed, title or tax record indicating the <br />land is owned by the City of Rifle. <br />10) Prior to any decision, staff will need proof of publication, an affidavit of publication and proof of <br />notice to all landowners and easements within 200 feet of the proposed permit boundaries. <br />This review is preliminary as noted. Responses to the Division's questions may raise more issues that <br />need additional information to resolve. Please note if the notices are done in error, the time frame for <br />comments starts anew. Additionally, Division staff will be conducting aPre-operational inspection. <br />During that inspection other issues may be noted that need to be addressed as well. The initial Decision <br />Date is set for June 7`h, 2007.
