In the interest of efficient processing, <br />please check the appropriate box to indicate which type of application y u are applying <br />for. 111 permits may be issued only under special circumstances. Be surfe you are <br />eligible. <br />[]Y LIMITED IMPACT OPERATIOtJS must disturb less than ten acres (in~luding all <br />roads, stockpile areas and any other surface facilities) over he life of the <br />mine, and must extract LESS THAN 70,000 tans of mineral, overb rd en or <br />combination thereof per calendar year. If the affected area is~ some fraction <br />greater than nine acres, be sure NOT to round this figure up to~ ten acres in <br />the narratives or elsewhere. ~ <br />[~ SPECIAL (ten-day processing) operations are any sand, gravel, o quarry <br />aggregates operations which are to be operated for the sole pur ose of <br />obtaining materials for highway, road, utility, or similar-type construction <br />under a Federal, State, county, city, town, or special district contract where <br />the contract calls for work to be Commenced within a specifical y short tine <br />and which will affect ten acres or less (including all roads, s ockpile areas, <br />and any other surface facilities). <br />INSTRUCTION: This application and all accompanying materials as outlinedlbelow must be <br />submitted in duplicate.
