6.0 Employee Train • <br />The SWMP Administrator or designated training personnel shall <br />review environmental and safety procedures with all personnel as <br />required by the Mining Health and Safety Administration. This <br />training session will be conducted to inform personnel of <br />procedures to be followed so that the operation is conducted in <br />compliance with permits. Some of the topics which may be included <br />are: <br />Spill Prevention and Response <br />Good House Keeping Techniques <br />Material Management <br />Sediment and Erosion Prevention <br />Crew supervisors will be responsible for instructing those under <br />their direction to implement the SWMP, so that the conditions of <br />the CDPS permit are achieved. <br />7.0 Testing and Control for Non- stormwater Discharges <br />If any unauthorized sources of non-stormwater discharges are <br />noticed at the site, the SWMP Administrator shall be notified <br />immediately. The source and condition of the discharge will be <br />investigated. If the discharge is perceived to be a potential <br />contaminant source to stormwater, the Administrator or designated <br />personnel shall notify the Colorado Department of Health. <br />SWMP CERTIFICATION <br />I certify under penalty of law that a complete stormwater <br />Management Plan, in compliance with Part I.C.2 of the Sand and <br />Gravel Mining general permit has been prepared and implemented for <br />my facility. Based on my inquiry of the person or persons who <br />manage the system, or those persons directly responsible for <br />gathering the information, the stormwater Management Plan is, to <br />the best of my knowledge and belief, true, accurate, and complete, <br />and implemented as written. <br />I am aware that there are significant penalties for falsely <br />certifying the completion of said SWMP, including the possibility <br />of fine and imprisonment for knowing violations. <br />Operator/SWMP Administrator Date <br />NOTE: The letter of completion and implementation of the SWMP <br />must be submitted to the Colorado Department of Health <br />with the stormwater Discharge Permit application.
