Hellner <br />RE: File No. M-87-050 <br />Adequacy Items <br />The following items must be addressed by the applicant in order to satisfy the <br />applicable requirements of C.R.S. 34-32-101 et sea. Mineral Rules and <br />Regulations of the Mined Land Reclamation Board- <br />Application Fee <br />An application fee of E350.00 is required unless the permit is solely for work <br />under a contract issued by the State Department of Highways (DOH). Please <br />furnish a copy of a DOH contract or the application fee. <br />Performance and Financial Warranties <br />A Performance Warranty and a financial warranty in the amount of E25,000.00 <br />must be submitted to the Division before a permit can be issued. Please <br />contact the Division to obtain a form for the type of warranty you wish to <br />use. A Performance Warranty is enclosed. <br />Exhibit ~ - Mining Plan <br />A detail~ad text of your proposed mining plan needs to be submitted. Please <br />include the following information: <br />1. The description and location of any permanent man-made structures <br />located ~Nithin 200 feet of the permit area. <br />2. Th.a depth and volume of topsoil, overburden and gravel. <br />3. Th.a type of equipment to be used during mining, please include any <br />processing equipment such as a crusher. <br />4. Will any ground water or surface water be encountered? If so, please <br />describe how ground or surface water will be protected from damage due to <br />mining. <br />5. How will erosion be controlled on the site? <br />6.
