Before <br />the request for this action submitted <br />on Nov. 15 was approved by the <br />Department. Portions of the roadway <br />lie outside of the operatofs approved <br />disturbance limit. <br />00-01-01 During a review of Decker's quartedy <br />MT: MDEQ blasting records for the 4th quarter <br />1999 and December 31, 1999, <br />Decker conducted blasting operations <br />after sunset. ARer being notfed of <br />the violation, Decker examined the <br />current blasting records and <br />discovered one additional Incident. <br />Decker did not in any of these cases <br />file a written report within three days <br />of the event, as required when late <br />shots must be detonated for safety <br />reasons. <br />00-07-01 Durng a review of Decker's quartedy <br />MT: MDSL blasting records for the 4th qtr. 1999, <br />Decker conducted blasting operations <br />after sunset. After being notified of <br />the violation Deeker examined the <br />current blasting records and found 1 <br />additional incident. <br />Last <br />Action Assessment Status Activity <br />Abated 11!18/1999 <br />11/18/99 -Based upon an inspection of the completed work by the <br />Department's Inspector no further abatement Is necessary. <br />Terminated 0 2/1 612 0 0 0 <br />Decker took immediate steps to conduct all future blasting in compliance with <br />the permit(i.e. from sunrise to sunset) and by February 17, 2000, initialed a <br />procedure which adequately informs all blasters of the current blasting <br />schedule limitations, anc submitted, for Department review, a summary of <br />that procedure. <br />Terminated 02/16/2000 <br />Decker took immediate stets tp cpmdict all future blasting in compliance with <br />the permit (i.e. sunrise to sunset) and by Feb. 17, instituted a procedure <br />which adequately informs all blasters of the current! blasting schedule <br />limitations, and submitted, for the Departmental review a summary of that <br />procedure. <br />Date MR-57 CF 4.1.1.3 <br />
