2.03.9 Personal Injury and Property Dama~Insurance
Information

(1) A certified copy of insurance for personal
injury and property damage protection for an
adequate amount to compensate any person
injured or property damaged as a result of
surface or sub-surface coal mining and recla-
mation operations, including use of exploxives.
Minimum insurance coverage for bodily injury
shall be $300,000 for each occurrence and
` $500,000 aggregate; and minimum insurance
coverage for property damage shall be $300,00
for each occurrence and $500,00 aggregate.
(2) The policy will be maintained in full force
during the life of the permit and any renewal
thereof, including completion of all recla-
mation operations under. Rule 4. '
(3) The policy will contain a rider requiring that
the TILE BLUE FLAME COAL COT4PANY will notify the
Divia~..,. wrrenever substative changes are made in
the policy, including any termination or failure
to renew.

(4) The BLUE FLAME COAL COMPANY, if acceptable, shall
submit satisfactory evidence of self-insurance
capabilities to the DeF~artment, if and when Blue
Flame would elect the method of self'-insurability.
