Berra. <br />Division DneClor <br />Re: File No.: M-90-087, Earl /Hoffman/Reyher, Incomplete Application <br />Dear Mr. Carder: <br />Upon reviewing your application for a Mining & Reclamation Permit, received <br />June 20, 1990, we have determined that it is incomplete under the Colorado <br />Mined Land Reclamation Act (C.R.S. 34-32-101, et seq.), and the associated <br />regulations. <br />The following items must be submitted in order for the application to be <br />considered complete (Rule 1.1(10) of the Rules and Regulations of the Mined <br />Land Reclamation Board, as amended, effective July, 1978): <br />Rule 3.2 (1)(d) requires a statement from the operator that the applicant <br />has applied for all necessary approvals from local government. <br />Rule 3.2 (1)(k) requires a list of alt other permits, licenses and <br />approvals which must be obtained from the federal, state, county or city <br />governments for conducting mining and reclamation operations (e.g., <br />NPDES, air pollution, radioactive source materials licenses, etc.) and <br />state the dates each was obtained, was applied for, will be applied for <br />or was denied; <br />If you have any questions, please do not hesitate to contact me. <br />Sincerely, <br />r <br />~, ~ <br />Gregg R.
