-4- <br />6. Provide a topographic map of the scale 1:6,000 or lazger (an enlazged USGS topo- <br />graphic map is acceptable) which cleazly shows: <br />a. The azea of land to be affected and the natural drainage above and below the <br />affected azea. <br />b. The names of property owners within the area to be affected and of the <br />adjacent lands. <br />c. The locations of existing structures and developed water resources within the <br />area to be affected, and on adjacent lands within %x mile of the permit azea. <br />d. The location of existing and proposed test borings or core samplings. <br />e. The location and extent of known workings of any underground mines within <br />the permit azea and adjacent azea. <br />7. Copies of documents must be attached with show that: <br />a. The applicant has a legal right to enter and commence mining within the <br />proposed azea (leases, sub-leases, and/or sale documents aze acceptable); and <br />b. A legal right of entry has been obtained for the Division of Mined Land <br />Reclamation, Office of Surface Mining and laboratory personnel to inspect the <br />lands to be mined and adjacent lands which may be affected to collect <br />environmental data or install necessary instruments. A Right-of Entry form <br />has been provided for your convenience. Use additional copies as necessary. <br />
