• <br />ADEQUACY REVIEW <br />for <br />Dorchester - Fruita Mine Numbers 1 and 2 <br />April 9, 1981 <br />2.03.9 Personal Injury and Property Damage Information <br /> <br />The applicant is advised that since the expiration dates of the insurance policy <br />are May and June, 1981, this policy will have to be reinstated prior to issuance <br />of a permit under the Colorado Permanent Program. Also, the policy shown is <br />insufficient in detail to determine whether the minimum coverage for bodily or <br />property damage have been met. <br />2.03.10 Identification of Other Licenses and Permits <br />1. Please supply the Division with an explanation of the present status of the <br />NPDES permit. It is noted that your previous permit expired on December 3, 1980. <br />Have you reapplied? <br />2. Have any decisions regarding a conditional use permit for the rail siding <br />been made? <br />2.04.3 General Requirements: Site Descriptions and Land Use Information <br />The map supplied is not a land use map. This map should delineate all lands <br />identified by the categories given in 1.04 under, "Land Use" (definition 71) and <br />should be prepared at a scale of not less than 1:24,000 as required by 2.10. <br />2.04.4 Cultural and Historic Resource Information <br />The archeological sites described in the report as occurring within two miles <br />of the study area should be located on the map. It is important to determine <br />whether any aspect of the mining operation could potentially impact these <br />resources. The State Historical Preservation Office currently is assisting us <br />on the review of this material. Further comments may be forthcoming. <br />2.04.6 Geology Description <br />1. The permit boundaries and mine workings (face-up area) should be indicated <br />on Map A-GEO and C-GEO. It is difficult to identify the proposed development <br />of the mine relative to these features. <br />2. The applicant should supply a regional geology map showing major lithologic <br />units and major fault systems in the area. <br />3. The coal analyses submitted by the applicant lack information regarding <br />pyrite. These data are required to satisfy section 2.04.6(2)(a)(iii)(c). <br />Additional data should be supplied to the Division to constitute a complete <br />chemical analysis of the coal. Please refer to the list of parameters in the <br />Appendix A. These parameters are required to assess the chemical nature of <br />the disturbed materials such that their potential effect on the quality of the <br />ground and surface waters can be realized. <br />4. The applicant has supplied lithologic logs for drill holes SC-1, SC-1C, SC-3, <br />SC-4 and SC-6 but locations are not given on Map A-GEO for these holes. Conversely, <br />
