Enclosed is a brief description and Bid Schedule to develop a backfilling and grading plan for a mine
site through the Office of Mined Land Reclamation for the State of Colorado using monies from the
Small Operator Assistance Program ISQAP). The State plans to issue a Purchase Order Contract to
complete the described work. The project schedule, a description of the selection method and terms
and conditions of the Purchase Order Contract follow below:

Submittal of Qualifications and Experience
All interested offerors must send completed Prior Experience form (attached) as well as information
detailing the qualifications of up to 3 personnel who will work on the OC #2 project. Qualifications
submittals shall not exceed 3 pages. Qualifications and experience documents should be faxed,
