The applicant should submit the Hanes and addresses of every officer, <br />partner, director, or other similar persons (2.03.4(2)(a)), names and addresses <br />.of any principal shareholder (2.03.4(2)(h)), and the names under which the <br />applicant, partner, or principal shareholder operated a coal mining operation <br />in the United States in the last five years (2.03,4(2)(c/). <br />Z. The applicant should submit a statement of all lands, inCerest in lands, <br />options, or pending kids on interests held or mar7e by the applicant for lands <br />which are contiguous to the perrut area (2.03.4(7)), <br />5.
