List by permit or permit application number and iden- <br />tify regulatory authorities. <br />Provide a MSNR number, if any. <br />Provide a list, if any, of contiguous lands, interests in lands, options, <br />or pending bids on interests held or made by the applicant for lands <br />which are contiguous to the area to be covered by the permit. <br />No proper certificate of liability insurance is submitted. <br />Provide a copy of the NPDES permit. <br />Land Use Information <br />A demonstration of the feasibility of land use for grazing must be made. <br />Geology Description <br />The core log provided (Plate 5) is not identified by number and is, for the <br />most part, illegible, therefore a legible, properly identified copy of the <br />log must be supplied. Additionally, chemical analyses of the overburden <br />and the underburden (roof and floor rock) including pryite content and po- <br />tential alkalinity must be supplied. If such analyses are not available <br />at this time, an explicit statement to this effect, including the pro- <br />jected date of availability, must be included. <br />A waiver to the requirments of paragraph (2)(a) of this section is in- <br />appropriate due to the unavailability of equivalent data despite a state- <br />ment to the contrary made on page 39 of the Mining and Reclamation Plan, <br />Hydrology Description <br />The source of the drill hole data should be stated. If a survey of existing <br />wells and springs in the mine plan area and the adjacent area has shown no <br />wells (beyond W-3050) or springs to exist, this fact should be so stated, <br />If wells or springs have been identified, as much information as possible <br />should be provided concerning them. Well data should include location, <br />yield, owners name, aquifer, depth to screened interval, and water quality. <br />Spring information should include location, flow rate, source aquifer, and <br />water quality. <br />Information on wells and sorinas is not only needed for a ground water <br />assessment, but also for alluvial valley floor determinations, for pre- <br />diction of possible subsidence impacts (renewable resources in areas <br />subject to possible subsidence), and for the requirements of 783.17 (al- <br />ternative water supply information.) <br />Describe the observed frequencies of flow in Hubbard Creek (oa)lemeral, <br />
