List by permit or permit application number and iden- tify regulatory authorities. Provide a MSNR number, if any. Provide a list, if any, of contiguous lands, interests in lands, options, or pending bids on interests held or made by the applicant for lands which are contiguous to the area to be covered by the permit. No proper certificate of liability insurance is submitted. Provide a copy of the NPDES permit. Land Use Information A demonstration of the feasibility of land use for grazing must be made. Geology Description The core log provided (Plate 5) is not identified by number and is, for the most part, illegible, therefore a legible, properly identified copy of the log must be supplied. Additionally, chemical analyses of the overburden and the underburden (roof and floor rock) including pryite content and po- tential alkalinity must be supplied. If such analyses are not available at this time, an explicit statement to this effect, including the pro- jected date of availability, must be included. A waiver to the requirments of paragraph (2)(a) of this section is in- appropriate due to the unavailability of equivalent data despite a state- ment to the contrary made on page 39 of the Mining and Reclamation Plan, Hydrology Description The source of the drill hole data should be stated. If a survey of existing wells and springs in the mine plan area and the adjacent area has shown no wells (beyond W-3050) or springs to exist, this fact should be so stated, If wells or springs have been identified, as much information as possible should be provided concerning them. Well data should include location, yield, owners name, aquifer, depth to screened interval, and water quality. Spring information should include location, flow rate, source aquifer, and water quality. Information on wells and sorinas is not only needed for a ground water assessment, but also for alluvial valley floor determinations, for pre- diction of possible subsidence impacts (renewable resources in areas subject to possible subsidence), and for the requirements of 783.17 (al- ternative water supply information.)
