I III IIIIIIIIIIIIIIII ~ ~~ July 14, 1986 Mr. Peter O'Connor Mined Land Reclamation Division 423 Centennial Building ~~~~ i 6 ~98~ 1313 Sherman Street Denver, Colorado 80203 Re: BLUE FLAME COAL MINE APPLICATION FORM Dear Peter, Blue Flame Coal Mine has revised points 15 and 16 of the application form to reflect acreage per the discussion under this topic of the Mined Land Reclam- ation Division's Adeouacy Review Letter No. 3. 2.03.4 Identification of Interests Mr. Huntington and the Ute Mountain Ute's are the legal surface owner's of record. This has been addressed in the application and the Blue Flame Coal Mine has sent certified letters to each, requesting what they intend to use the land for after Blue Flame Coal Mine has reclaimed the mine surface pursuant to Rule 2.05.5(2)(b). As of this time, Blue Flame has not received any correspondence from them. Please see the attached copies of said letters. 2.03.9 Personal Injury and Property Damage Insurance Information 1. Blue Flame Coal Mine has notified our insurance agents to revise the rider that appears on the latest copy you have received. They have committed to notifying the Mined Land Reclamation Division if any changes are made it the insurance policy or any failure to renew occurs. 2. Blue Flame Coal Mine will revise all pages where changes were made during the adequacy review process. It is understood the Division will not inter- lineate the words requested. Blue Flame Coal Mine commits to revising these changes and omitting all revised changes where changes have been made prior to final approval of the permit application. 2.04.7 Hydrology Description 1. Blue Flame Coal Mine has committed to monthly water quantity and to quarterly water quality sampling. Blue Flame Coal Mine takes the Division's sugges- tion that Blue Flame Coal Mine eliminate the timetable and allow the text to stand on its own with the specified commitments. Blue Flame Coal Mine also accepts the Division's suggestion to submit water quantity measurements along with water quality measurements on a quarterly basis. 2. Blue Flame Coal Mine has depicted the location of the 500 gallon portable plastic water tank on Map 1. As indicated previously, this 500 gallon plas- tic water tank will be portable and most likely left in the back of a pick-up.
