Box 1425 <br />Durango, CO 81301 <br />Re: PAR for Blue Flame Mine <br />File No. C-053-81 <br />Dear Mr. Lobato: <br />As a result of our Preliminary Adequacy Review of your permit application, the <br />Division has identified the following items which need to be provided to us <br />before we can complete our review. <br />1) Two more copies of your complete application must be sent to us. <br />2) The site drawing and topography map previously submitted to us must <br />have your permit boundary drawn on it. Also, all adjacent mineral <br />and surface owners must be identified on the map. When this is <br />done, three copies of the map must be sent to us. Each map must be <br />certified by a qualified registered professional engineer. One <br />copy must have an original signature by the engineer. <br />After we receive these items, we will continue to review your application.
