These <br />items were previously referenced in the letter and responses submitted to the Division, dated May <br />1, 2007. Responses were prepared by Environmental Alternatives, Inc., and Weaver Engineering <br />Services. <br />Adequacy Review questions dated November 28, 2006, per question number: <br />6. Personal injury and property damage insurance certificate is provided .Please insert in <br />reserved Exhibit AD, in Volume 3 of the permit application. <br />24. The Vegetation Map (Map 3) was prepared by Kent Crofts of IME Consulting and reflects <br />vegetation study of the proposed disturbed site. Please replace existing Vegetation Map, <br />by Betty Miller, with the enclosed IME map, in Volume 1 of the permit application. <br />Adequacy Review questions dated December 15, 2006, per question number: <br />19. Mine facilities details were removed from Map 1. A new facilities map, Map 12, is <br />provided. Please insert in sleeve for reserved Map 12, in Volume 1 of the permit <br />application. <br />Additional permit modifications as a result of the aforementioned adequacy issues include: <br />Exhibit V, Drainage and Sediment Control Plan, required changes to include 100-year <br />storm event flow designs for ditches as a result of adding the refuse pile structure. A <br />revised plan is provided replacing the existing plan in Exhibit V. Please replace the entire <br />existing Exhibit V with the enclosed plan, in Volume 3 of the permit application. <br />Map 11, Postmining Topography Map, was edited to reflect changes in the reclaimed <br />topography as a result of adding the permanent waste disposal pile.
