An officer of your company needs to
sign both copies and have them notarized. One original must be returned to
the Division at the address below. A copy must be kept on file at the mine
office.
This permit will not become effective until the Division has received and
accepted a reclamation bond in the amount of $53,000.00. The company should
be aware that issuance of the permit may initiate the timeframes of enclosed
stipulations to the permit. Stipulations are enclosed in the Rider attached
to the permit. The Rider form must also be signed by an officer of the
company.
All other enclosed commitments are also in effect as of the permit issuance.
If you have any questions, please contact me.
Since 1y,
`~ ~ ~lb^
8erhan Keffelew
Reclamation Specialist
Enclosure(s)
0695G
