items. The application may not be processed until complete.
1. Name of the owner(s) of the surface of the affected land: United States Steel Corp.

Name of the owner(s) of the subsurface rights of the affected ]and:
United States Steel owns all mineral rights except the "E" coal seam.
3. A map showing information sufficient to determine the location of the affected land
on [he ground and existing and proposed roads or access routes to be used in connec-
tion with the mining operation. This map should be of a scale sufficient to clearly
show the affected land and should also show topographic lines sufficient to reveal
the direction and slope of the land. Label Exhibit A.
4. Water resources: A brief description of:
a) the surface water occurring on the affected land;
b) the subsurface water occurring on the affected land;
c) the estimated water needs for the operation; and
d) the need for a NPDES permit. '
NOTE: If no water occurs on the site state such.

$. Wildlife resources: A brief statement listing the species of wildlife occurring on
the affected land, noting any endangered or threatened species, and a su-^iary of the
influence of the operation on the existing wildlife. Consultation with the local
wildlife conservation officer of the Colorado Division of Wildlife is advised.
