to the mid-term renewal date. <br />-Zf at the mid-term review or during the permit renewal <br />the salvage information is not updated, or otherwise found to be <br />inadequate, the reclamation cost estimate will be revised and the <br />operator shall be required to submit additional bond within sixty <br />(60) days of notification. <br />-In order to identify and resolve concerns between <br />submittal of salvage credit materials, the Division shall notify <br />the operator of any conditions which are observed which could <br />potentially decrease the value of the facility. Likewise, should <br />the permittee become aware of similar circumstances, they shall <br />promptly notify the Division of such occurrence. <br />-All structures and facilities designated to be utilized <br />as salvage credit shall, prior to acceptance by the Division, be <br />insured to protect against fire or other loss, to at least the <br />amount of salvage value agreed upon by the Division and the <br />permittee. The certificate of insurance shall name the State of <br />Colorado, Division of Minerals and Geology as beneficiary of the <br />policy. <br />Please either include a cost for removing the track or comply with <br />the policy mentioned above. <br />4. Please include a cost <br />disposal for material that may be <br />quantity of 50 gallon drums <br />$520.00/drum. <br />for hazardous waste testing and <br />located on the site. Include the <br />using the MEANS unit cost of <br />5. Peabody mentioned that concrete will be disposed on the <br />HG Facility Mine site. Please include a cost for disposing this <br />material on the site. MEANS 1993 estimates that on site disposal <br />of concrete is $5.06/cy. <br />6. Please indicate if PCB transformer removal is necessary <br />at the site. If PCB material is evident on site, please include a <br />cost for testing, removal and disposal of the material. <br />7. It appears that Peabody did not include a location <br />adjustment factor for removal of the facilities. In Colorado the <br />MEANS location adjustment factor is 102.50%. Please include this <br />factor in your estimate. <br />8. Please include a cost for removing miscellaneous <br />materials, supplies and debris that may be located on the site <br />prior to reclamation. <br />Mobilization/Demobilization <br />Peabody states that mobilization/demobilization of equipment will <br />be 5$ of the direct cost. The Division calculated the total cost <br />of mobilization/demobilization on the estimating form attached. <br />
