An officer of your company <br />needs to sign both copies and have them notarized. One original must be <br />returned to the Division at the address above. The copy must be kept on file <br />at the mine office. <br />The company should be aware that issuance of the permit may initiate the time <br />frames of enclosed stipulations to the permit.
