No references for technical or written material are given in the report even though citations are given in the text. A list of references used or cited should be supplied to satisfy 2.03.3(1). 2. An original signature by a responsible official of the applicant should be provided in each application as stated in 2.03.3(7) and (8). 3. It is unclear what laboratory performed the coal analysis in Table 2.1-2 as required by 2.03.3(3). 4. Many of the maps are not of reproducible quality as required in 2.03.3(5). Maps showing drainage of the permit area and other hydrologic information should be of particularly good quality. Please re-submit these maps. 2.03.8 Permit Term Information The application does not give particular starting and termination dates for phases of mining. 2.03.9(1) Personal Injury and Property Damaoe Insurance Information Minimum liability for bodily injury and property damage is required. Since this information was given only for the aggregate in the application, it is impossible to determine whether these minimum amounts have been met. 2.03.10 Identification of Other Licenses and Permits The applicant should supply identification and information of any NPDES or special land use permits to comply with this regulation. The Division would request notification of any delays, denials or problems associated with the obtainment of these permits. 2.03.12 Newspaper Advertisement and Proof of Publication The Division has not yet received proof of publication notice as required. 2.04.3 Site Description and Land Use Information 1. Vegetation map 4.5-1 in the application shows an agricultural area which does not appear on the land use map (4.1-1) and vice-versa. This information may be necessary to make an AVF determination. A narrative on land capability and productivity required by 2.04.3(2)(b) should be completed in this section. 2.04.4 Cultural and Historic Resource Information 1. An explicit statement that all areas of potential surface disturbance have been 100% surveyed is needed, ~•~ith exact areas of survey shown on maps, and concise legal descriptions and acreage of inventory. 2. Amore detailed explanation of sample selecting procedures is needed.
