<br />postotfico address, the name of the claim or claims to bo operated. the name of the crouaty and mining district,
<br />togother with the number o[ men employed, directly or indirectly, rho sumo being cleest6ed into minors, tram-
<br />ntars, timbormon, oro aeenrters, mill men. toametere, etc. Tho nreaeeary blanks to carry out rho pcovieione of
