<br />together with the names at the ownere aD mansgare or leeaee in charge of said work, together with the
<br />poetn8lro address, the name of the claim or claims to bo o~orated. the Dnme of the county and mining district,
<br />together with the number n( mea employed, directly or ndirectly, the same being claeelfled into minere, tram•
<br />mere, timberman, aro aseortere, mill men, teamatere, etc. Tho necessary blanks to carry out the provietona at
<br />this section shall bo famished upon epplicetioD by the Commissioner of Mines.

<br />ngafsat this act, and, when not otherwise provided, shall bo liable to the penalty preecribed in section 13 of this
<br />act, or to a ffna of not leas than twontyffve dollare (1'ffi.001, nor more than three 6undrod dollars (1300.00), for
<br />such snd ovary provision not compiled with, or both, at the diecrotioD of the court. '
