• (Page 4) <br />MINE ID # OR PROSPECTING ID # H~~~IQ S <br />INSPECTION SATE ~Zi)~1A INSPECTOR'S INITIALS VCs <br />The following problems (PB) and/or possible violations (PV) (and suggested <br />corrective actions) were identified during this inspection. The problems <br />should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given <br />to reduce their severity when considered by the Mined Land Reclamation Board. <br />The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />1. The currently posted Financial Warranty of $5,000.00 <br />appears insufficent to complete reclamation of the <br />proposed pit. <br />CORRECTIVE ACTION:The Division will re-evaluate the <br />adequacy of the Financial Warranty. If it is <br />determined to be insufficent, the permittee will be <br />notified and required to post additional bond. <br />2. Water continues to seep from the base of the highwall <br />and to pond, to a limited extent, on the pit floor. <br />If this ponding involves the permanent exposure of <br />groundwater, there is a conflict with both the <br />approved mining and reclamation plans. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />Within 60 <br />days of a <br />notice of <br />insufficency. <br />PB or PV:PB <br />TOPIC(S):FN <br />April 1, 1999 <br />PB or PV:PB <br />TOPIC(S):HB <br />CORRECTIVE ACTION: The permittee must determine and <br />report the source of this water and the anticipated <br />long term relationship of this water to the pit.
