_• `;
(Page 2)
NINE ZD f OR PROSPECTING ID f N-80-244
INSPECTION DATE 9 20 94 INSPECTOR'S ~I NITIALS ACS
OBSERVATIONS
An inspection was conducted of the Creeeon Project Phase 1 leach pad construction.
A persistent light rain had occurred at the project site on the night preceding this
inspection. As a result, the soil liner that had been placed but not covered would have to
be re-compacted. Since the placed soil liner obviously exceeded the permit limits of 3
percent above optimum moisture content for compaction, there was no soil Liner work occurring
on this day. No geomembrane work was being conducted at the time of this inspection. The
contractor had installed some crescent shaped ditches in the soil liner below the VLDPE lined
areas in an effort to minimize erosion of the soil liner caused by the rain.
The only significant earth-work occurring at the site this day was finish grading of the toe
berm, and placement of the select structural fill shell.
The up-gradient Arequa Gulch sump was inspected. Some sediment was deposited against the
straw-bales surrounding the sump as a result of the rain storm; all Arequa Gulch flow was
being passed through straw-bales prior to entering the sump. The process plant access road
fill has encroached onto the edge of the sump, and has rendered the existing bales incapable
of controlling sediment from the access road berm. It was agreed that the straw-bales would
have to be stacked higher around the entire sump, and maintained such that any influent to
the sump would pass through the bales, rather than over or around them. See PB-1 on the
final page of this report.
The underdrain outlet below the toe berm was inspected. The water issuing from both the 24"
solid and the 24" perforated pipes was observed to be clear.
Since no soil liner placement or compaction, nor any geomembrane welding was going to take
place this day, the inspection was concluded.
cc: Berhan Keffelew, DMG
Carl Mount, DMG
I & E Contact Address
~ CE
NAME John Hardaway ~ BL
OPERATOR Cripp le Creek 6 Victor Gold Co. ~ FS
STREET P.O.
